The Merit-based Incentive Payment System (MIPS) came into effect on Jan. 1st, 2017. It is one of two tracks under the Quality Payment Program (QPP) of CMS, that brings Medicare Part B providers to a performance-based payment system. The QPP replaces three previous programs, Meaningful Use, PQRS and the Value-based Payment Modifier, with one new program with four performance categories: Quality, Cost, Improvement Activities, and Advancing Care Information (ACI).

Based on Composite Performance Score (MIPS Score) earned on the measures reported, eligible clinicians earn a performance-based payment adjustment for Medicare payments starting in 2019. While MIPS is designed to be a budget-neutral program, there is $500 million allocated to provide additional incentives to exceptional performers.

A core requirement for future reimbursement is the completion of a security risk analysis, including the remediation of areas not in compliance.
